brain injury : Care Pathways

Our Care Pathways for the Brain Injury Service

An important feature of this service is its range of care pathways allowing service users to progress through the appropriate stages within a single service. Service users can be admitted to, or discharged from, any stage in the care pathway.

Access to the range of rehabilition inputs necessary is given according to individual assessment. Skills are developed through practice, guidance and feedback, aimed at recognising achievements and successes. This is achieved through a combination of therapeutic treatment and community-based activities to encourage independence and minimise any challenging behaviour. Creating such an environment combined with staff skilled in creating positive outcomes out of negative situations helps us to achieve renowned and notable results.  We are able to deliver a range of outcome measures that evaluate progress from admission through to discharge.

A behavioural management system is followed through which service users receive a comprehensive individual rehabilitation programme, which involves both individual and group sessions.
