*Admission Process
SRVdivADD_bullet*Screening of all referrals is completed by the Responsible Medical Officer and Clinical Team against the relevant Admission Criteria. Click here for Service Summaries.
SRVdivADD_bullet*Contact is made with family (where applicable)/carers and Social Worker.
SRVdivADD_bullet*Two Members of the Clinical Team arrange an assessment visit.
SRVdivADD_bullet*Following assessment a decision regarding admission is made by the Clinical Team.
SRVdivADD_bullet*Following acceptance Initial Care Plan and Risk Assessment is completed prior to admission, including allocation of Keyworker and named nurse. This includes any special nursing requirements.
SRVdivADD_bullet*When a bed is available the preparation for admission will usually take 2-3 weeks.
SRVdivADD_bullet*Please refer to Admission Criteria given within the Service Summaries for further information.
SRVdivADD_bullet*Risk Assessment reviewed within the first 4 weeks of admission.

Assessment
The initial assessment period is 16 weeks and involves individual assessments carried out by appropriate members of the team. This includes core assessments by:

SRVdivADD_bullet* Psychology
SRVdivADD_bullet* Occupational Therapy
SRVdivADD_bullet* Social Work
SRVdivADD_bullet* Education
SRVdivADD_bullet* Speech and Language Therapy
SRVdivADD_bullet* Basic health check

Other therapies include:

SRVdivADD_bullet*Physiotherapy
SRVdivADD_bullet*Art Therapy
SRVdivADD_bullet*Psychotherapy

The first Risk Assessment is reviewed and any amendments made. Each team member reports on his or her findings and an initial multi-disciplinary care plan is formulated in preparation for the first Clinical Review. The patient's first clinical review takes place at the end of the assessment period and a decision is made whether the individual's needs can be met on the unit.
