| |  |  |  |  | Communication |  | |  | |  |  |  |  | Multidisciplinary Care Planning Meetings These meetings are held prior to a clinical review to ensure that our reports are consistent and accurate. The existing plan and risk assessment are circulated in advance and staff submit modifications for the meeting.
Clinical Reviews/CPA and Section 117 Meetings The purpose of the CPA Review is to agree upon a multi-disciplinary Care Plan, discuss progress, future aims and continued suitability for the unit. These are arranged on usually a 4 monthly basis, the first meeting, the Assessment Review, being at the end of the initial 16 week assessment period. Normally Reviews are attended by the Clinical Team, Business Manager and the patient (and advocate, if desired) and invitations are sent to relatives/carers (if appropriate), referrers, purchasers and Social Services. In order that St Andrew's Hospital and home agencies can plan and work together it is important that representatives attend these meetings and this commitment is expected as part of our Service Agreement.
User Involvement Meetings Each service communicates with its patients through "User Involvement Groups" or Community Meetings. The purpose is to empower the patients to participate in decision making regarding their care and environment.
Intervention Planning Meetings These meetings are called by any member of the clinical team as required to discuss particular aspects of a patient's care (eg ground leave, incentive programme, risk assessment, etc). These meetings should be called as quickly as possible and must involve the named nurse. The external agencies are not invited to these meetings. Any decisions and changes to a patient's care plan will then be incorporated into their care plan and discussed at the next care planning meeting.
Statutory Meetings required by the Children Act 1989 These meetings are called and chaired by the child's local authority. St Andrew's Healthcare staff will attend and make a verbal contribution but written reports are not produced.
Ward Rounds These meetings are held weekly and are chaired by the Responsible Medical Officer. Patients are discussed according to a scheduled rota. Records are made of the ward round.
Clinical Management Team Meetings These meetings are held on a regular basis and discuss fixed agenda items such as accidents and incidents, sickness and absence, as well as other management issues such as policies and procedures. A patient representative from the User Involvement Group can attend if necessary. Notes of these meetings are circulated to staff.
Performance Indicators The service presently uses the Adaptive Behaviour Scale to measure each patient's progress in performance and a copy is included in the clinical review documentation. In addition, the risk assessment documentation assesses the level of risk within the treatment programme. A copy of this is also included in the clinical review documentation. We are pleased to work with purchasers to determine what additional performance indicators may be required.
Complaints and Advocacy Patients are supported to make a complaint if they feel that their concerns have not been appropriately dealt with. Any complaints are formally entered in to a ward register and are acknowledged within 3 days and actioned within 10 days. The hospital has a liaison officer who can visit the patient and deal with any investigations that may result. St Andrew's Healthcare uses the independent advocacy service Speaking Up. Patients are encouraged to make use of these services. The advocate may assist the patient in making a complaint and will represent their views. Advocates may attend CPA meeting.
Mental Health Act Requirements All patients are encouraged to make use of the Mental Health Act Commission patient focused visits. All patients are informed of their rights under the Act. Work is ongoing at providing visual guides for our patients with comprehension and reading difficulties. All patients are informed of the process of Tribunals and they are encouraged to be supported by a befriender or advocate who is then able to interpret what has been said. Tribunal decisions and their future in St Andrew's Healthcare are discussed with the patient. In accordance with the Mental Health Act, all patients can attend relevant Managers meetings and are encouraged to take an active part, supported by staff.
Patients' Charter St Andrew's Healthcare has its own patients' charter. A copy is given to all patients on admission.
Clinical Audit The division is actively involved in auditing our clinical work to improve quality standards.
