| |  |  |  |  | Referral and Admission |  | |  | |  |  |  |  | All referrals are acknowledged in writing no later than the day after the date of receipt. Any additional information required is requested as part of the acknowledgement letter.
Within seven days of receipt of the referral or if required, the additional information, Clare House will provide a provisional date for an initial assessment of the patient.
To ensure maximum effectiveness from the referral process an up to date risk assessment should be provided within the referral data together with the latest CPA care plan, nursing, OT, Psychology reports and copies of medical or social work reports.
Specifically, the CPA care plan should include details of the aftercare to be provided in the event of discharge from hospital including the following:
 Management of the patient’s illness
 The patient’s ability to develop and sustain relationships
 Opportunity for training skills development/employment opportunities
 Accommodation needs and the patient’s ability to live independently
 The patient’s ability to perform daily living tasks
 Arrangements for medication
Assessments On the day of an assessment the referring unit should ensure that the patient’s named nurse/key worker or social/community worker are available to provide such additional or background information as may be required.
Decisions An in-principle decision is usually sent in writing no later than seven days after the date of assessment.
Transfer Arrangements Upon confirmation of the intention to transfer to an identified bed a period of two weeks is usually adequate to facilitate transfer. At this point it is helpful if section papers are faxed to the Clare House safe haven fax on 01268 723843. However, as an alternative to faxing, copies of the section papers can be provided on the day of the pre-admission visit.
The referring unit must take all necessary steps to ensure that Home Office approval is granted in a timely manner.
A pre-admission visit by Clare House nursing staff will be arranged within five working days of a positive decision to admit. This visit will be primarily to conduct a baseline assessment but also to consider certain practicalities such as:
 The need for specific reporting arrangements in event of escape or absconding
 Dietary needs, arrival time on date of transfer and other practicalities
 Storage requirements for the patient’s belongings
 Visiting arrangements for friends and relatives including children
Wherever possible pre-admission visits to Clare House by the patient, relatives and carers is encouraged.
In terms of transit arrangements it is usual for transport and escorts to be provided by the discharging unit.
On the day of transfer the discharging unit should provide benefit books, medicine charts and the patient’s most recent CPA care plan (in accordance with S117). The original section papers must also accompany the patient.
Within one week of transfer the discharging unit should provide a discharge summary for the patient together with any other information as reasonably requested by Clare House.
