Overview An overall Risk Management Structure provides a framework within which personal safety is paramount. Inside this structure an integrated, multi-system therapeutic and educational programme offers a range of interventions. Individual care plans are derived from an initially extensive and then ongoing assessment process using the principles of the Care Programme Approach. Liaison with referring agencies or other relevant local professionals is maintained from assessment through to discharge by periodic Clinical Review and pre-discharge planning meetings.
Risk Management Structure Clinical risk is assessed and reviewed at each stage of the young person's contact with the Lowther Adolescent Service from the pre-admission stage onwards. Risk factors are reviewed at regular intervals throughout the admission and at any time when indicated by a change in mental state, behaviour, setting or in response to transitions or life events. The Risk Management Structure facilitates and supports the provision of treatment, education and care. As the young people develop greater capacity for self-management of behaviour they may move through a hierarchy of levels within the Risk Management Structure towards increasing opportunity and responsibility.
Assessment During the first 10 weeks of an admission the multi-disciplinary team carry out an assessment of the young person's problems and needs using standardised assessment instruments, the developing relationship with the young person, contact with carers and reports from relevant agencies. The resulting profile of needs, risk factors and strengths enables the team, together with the young person, to develop a range of Care Plans. Each young person has a named Keyworker who co-ordinates this process within the principles of the Care Programme Approach.
Treatment Modalities The Lowther Adolescent Service provides a wide range of therapeutic modalities as well as education and recreation. The treatment approach combines psychiatric, developmental, family systems and cognitive behavioural perspectives integrated through regular meetings of a CPA Team with the young person. Individual and group therapies on psychodynamic, cognitive behavioural and creative therapy principles are available to all young people in the service. Focused work for abuse survivors and for young offenders is provided. Treatment aims begin by focusing on the management of mental illness and dangerousness and move through the development of appropriate coping skills towards goals expressed in terms of self management and autonomy.

Education Education sessions are provided in response to the assessed need of each young person through individually planned programmes. They are encouraged to achieve formal educational qualifications that are recognised outside the hospital. Our Education Department liases with referring Local Education Authorities and agencies and aims to work within a Statement of Special Education Needs should this apply. Support for young people attending other educational establishments or work experience placements is provided.
