 Screening of all referrals is completed by the Responsible Medical Officer and Clinical Team against the relevant Admission Criteria. Click here for Service Summary.
 Contact is made with family (where applicable)/carers and Social Worker.
 Two Members of the Clinical Team arrange an assessment visit.
 Following assessment a decision regarding admission is made by the Clinical Team.
 Following acceptance Initial Care Plan and Risk Assessment is completed prior to admission, including allocation of Keyworker and named nurse. This includes any special nursing requirements.
 When a bed is available the preparation for admission will usually take 2-3 weeks.

 Risk Assessment reviewed within the first 4 weeks of admission.
Assessment The initial assessment period is 16 weeks and involves individual assessments carried out by appropriate members of the team . This includes core assessments by:
 Psychiatry
 Nursing
 Psychology
 Occupational Therapy
 Social Work
 Education
 Speech and Language Therapy
 Basic health check
Other therapies include:
 Dietitian
 Physiotherapy
 Art Therapy
 Psychotherapy
The first Risk Assessment is reviewed and any amendments made. Each team member reports on his or her findings and an initial multi-disciplinary care plan is formulated in preparation for the first Clinical Review. The patient's first clinical review takes place at the end of the assessment period and a decision is made whether the individual's needs can be met on the unit.
