Admissions to the Huntington's Disease Service

St Andrew’s Healthcare’s Huntington’s Disease service accept men and women over the age of 18 years at all stages of the disease, and who require a low secure environment. We accept those detained under the mental health act (1983).

Referrals can be made by contacting our admissions office: 0800 434 6690 a named Consultant Psychiatrist or the Medical Director. A copy of the referral form is also available to download here. We acknowledge referrals within seven days of receipt.

A multi-disciplinary team is provided with several pre-admission templates, from which their initial assessment is made. These include:

  • St Andrew’s Healthcare service admission document
  • Multi-disciplinary care plan
  • Risk assessment
  • HoNOS
  • Social and personal history
  • Medication/medical history
  • Physical health
  • Forensic history

Upon admission, a 16-week multi-disciplinary assessment will form the core component of planning the care pathway. This identifies service user needs and establishes specific individual assessment criteria, which includes interventions and anticipated outcomes. Our care co-ordinators play a vital role in the implementation of this plan and recording any outcomes. Core assessments involve:

  •   Psychology
  •   Occupational Therapy
  •   Social Work
  •   Education
  •   Speech and Language Therapy
  •   Basic health checklist
