Admissions to our Learning Disability Service
Our learning disability service caters for men and women, aged 18 to 50 years and adolescents with mild or borderline learning disability and may have forensic needs, many of whom have a mental illness. Most will have been detained under the Mental Health Act (1983). Often, our residents are in need of specialist assessment and treatment programmes and may have failed to progress in previous environments.

A multi-disciplinary team is provided with several pre-admission templates, from which their initial assessment is made. These include:
- St Andrew’s Healthcare service admission document
- Multi-disciplinary care plan
- Risk assessment
- HoNOS
- Social and personal history
- Medication/medical history
- Physical health
- Forensic history
Upon admission, a 16-week multi-disciplinary assessment will form the core component of planning the care pathway. This identifies service user needs and establishes specific individual assessment criteria, which includes interventions and anticipated outcomes. Core assessments involve:

- Education
- Speech and Language Therapy
- Basic health checklist
