Services for adolescents : Admissions

Admissions to our Services for Adolescents

Our service cares for young people between 13 and 18 years of age on admission with mental health or learning disability issues including those with developmental disabilities and challenging and associated offending or high risk behaviours. This includes young people detained for treatment under the Mental Health Act (1983) and those with an offending history.

Referrals can be made via St Andrew's Healthcare Admissions Office on 0800 434 6690. Alternatively referral forms are available to download here

A multi-disciplinary team is provided with several pre-admission templates, from which their initial assessment is made. These include:

  • St Andrew’s Healthcare service admission document
  • Multi-disciplinary care plan
  • Risk assessment
  • HoNOS
  • Social and personal history
  • Medication/medical history
  • Physical health
  • Forensic history

Upon admission, a 16-week multi-disciplinary assessment will form the core component of planning the care pathway. This identifies service user needs and establishes specific individual assessment criteria, which includes interventions and anticipated outcomes. Core assessments involve:

  •   Psychiatry
  •   Nursing
  •   Psychology
  •   Occupational Therapy
  •   Social Work
  •   Education
  •   Speech and Language Therapy
  •   Dieticians
  •   Basic health check list
