Admissions to our Service for Older People
Our services are designed for those over the age of 55. Occasionally slightly younger people may be admitted providing their care and quality of life needs can be met on the unit. Where a person under the age of 55 is housed with those predominantly over the age of 55, a specific care plan will be available which addresses age-related issues. Those admitted will have mental health problems and require assessment and management in a secure environment. We can also accept service users with an offending history.
Referrals can be made via St Andrew’s Healthcare’s Admissions Office on 0800 434 6690. Alternatively a referral form is available to download here.
A multi-disciplinary team is provided with several pre-admission templates, from which their initial assessment is made. These include:
- St Andrew’s Healthcare service admission document
- Multi-disciplinary care plan
- Risk assessment
- HoNOS
- Social and personal history
- Medication/medical history
- Physical health
- Forensic history
Upon admission, a 16-week multi-disciplinary assessment will form the core component of planning the care pathway. This identifies service user needs and establishes specific individual assessment criteria, which includes interventions and anticipated outcomes. Core assessments involve:
- Psychiatry
- Nursing
- Psychology
- Occupational Therapy
- Social Work
- Education
- Speech and Language Therapy
- Dieticians
- Basic health check list
