The dementia and challenging behaviour service at St Andrew’s Healthcare accepts adults primarily over the age of 55, who are suffering from dementia and have complex needs including early onset dementia and acquired, static or progressive dementias.
Those younger than 60 with early onset dementia may also be considered for admission. We accept those with challenging behaviour that require a low secure environment.
Referrals can be made by downloading a referral form here or by contacting our admissions office on 0800 434 6690. We acknowledge referrals within seven days of receipt.
A multi-disciplinary team is provided with several pre-admission templates, from which their initial assessment is made. These include:
- St Andrew’s Healthcare service admission document
- Multi-disciplinary care plan
- Risk assessment
- HoNOS
- Social and personal history
- Medication/medical history
- Physical health
- Forensic history
Upon admission, a 16-week multi-disciplinary assessment will form the core component of planning the care pathway. This identifies service user needs and establishes specific individual assessment criteria, which includes interventions and anticipated outcomes. Core assessments involve:
- Psychiatry
- Nursing
- Psychology
- Occupational Therapy
- Social Work
- Education
- Speech and Language Therapy
- Dieticians
- Basic health check list
