Adult Care services 

Bespoke community services for people with complex needs 

We work with commissioners and other funding and care partners to develop bespoke community services for people whose complex needs cannot be appropriately met through existing provision. 

We can tailor an existing environment or work with partners to develop a new community-based setting, supported by a specialist multidisciplinary team providing 24-hour care. 

Our aim is to create sustainable community placements that provide the right level of specialist support while enabling people to live as independently as possible. 

Our Adult Care Services 

We currently offer Adult Care services in Northampton and Winslow in Nottinghamshire.  

Northampton - we offer four homes on The Avenue with a total of 11 beds. 

Winslow - we offer a residential building in Bestwood village, comprising six single occupancy apartments. 

Each resident benefits from independent living skills and community engagement in a calm and homely environment, whilst still receiving the support they need.

Our specialist Adult Care offer 

We can develop bespoke services for people with a range of complex needs, including: 

• Deaf people with complex mental health needs - Specialist community support designed around the individual's mental health needs, communication requirements and wider needs as a Deaf person.
• Complex neurobehavioural needs - Bespoke environments and specialist multidisciplinary support for people whose neurobehavioural needs require a highly individualised approach to care and risk management.
• Complex learning disability and autism needs - Community-based services tailored to people with complex learning disabilities and autism, with the environment, staffing and support model designed around their individual needs.
• Complex mental health care needs - Specialist community services for people with complex mental health needs who require a high level of support to live safely and sustainably outside hospital. 

Developed with commissioners 

We work collaboratively with commissioners, Provider Collaboratives, funding teams and other stakeholders to understand the individual's needs and develop the right solution. 

This can include: 

• Tailoring an existing community environment 
• Developing bespoke accommodation 
• Creating a dedicated multidisciplinary team 
• Agreeing the staffing and specialist expertise required 
• Developing the care model around the individual's needs 
• Agreeing the service and funding arrangements upfront
